remove applicaton for and location,
just have the applicant fill in the organisation that needs to be replaced with the word park
and the role
the header should also read 'MEMBERSHIP APPLICATION@

need to change
1. the logo
2. the header needs to read MEMBERSHIP APPLICATION in the same color as the logo
3. we have no more organisations, we only have parks, change the word from organsiation to Park
4. remove appicatoin for and location
